Please read the instructions carefully and follow them in filling out the application as best you can. If an application asks about governmental benefits such as SSI or Husky C, please provide accurate information.

If you have questions about your governmental benefits and/or your eligibility to receive the funds applied for and continue to be eligible for those governmental benefits, the Trustee will not be able to give you legal advice. You should consult with New Haven Legal Assistance, Maritime Center, 555 Long Wharf 9th Floor, New Haven, CT 06511, phone 203-912-0663 or personal lawyer. If you need a lawyer for advice on your benefit eligibility, please write, email or call the Trustee. The Trustee will try to help you find a lawyer to advise you.
Please note that the Trustee is only able to process your application on the information you provide. The Trustee has no responsibility or liability if the payment of any settlement amount results in ineligibility for any governmental program.
